These Terms are important, contain legal obligations and affect your legal rights, so please read them carefully.  For the avoidance of doubt, these Terms do not apply to your receipt of healthcare services from the Practice. BY ENTERING, ACCESSING, BROWSING, SUBMITTING INFORMATION TO, OR OTHERWISE USING THE SITE, YOU ACKNOWLEDGE AND AGREE TO THESE TERMS.  IF YOU DO NOT AGREE TO THESE TERMS, YOU MAY NOT ACCESS OR USE OUR SITE.

Privacy Policy:

The Privacy Policy is incorporated and made part of these Terms. You hereby agree to such Privacy Policy. Any personal information (for example, your name, date of birth, address, telephone number or email address) that you transmit to the Site will be used by the Practice in accordance with the Privacy Policy. Specifically, when using the Site to schedule an appointment, you agree that the information provided through the “Schedule Your Appointment” webpage may be collected and used for the purposes set forth in the Privacy Policy, including, without limitation, for analytics and advertising purposes, irrespective of whether the scheduling form was only partially completed or the scheduled appointment was thereafter canceled.

Refund / Shipping Policy: 

  • After a refund request is received, the seller should respond within 3 business days. 

  • Buyers have 30 calendar days after the estimated or actual delivery date to request a return, or 7 calendar days after the event date, whichever is later. 

  • Seller must give 24 hour notice if product or service is no longer available to customer.

    Shipping policy is not applicable as only services are rendered at this time.
